With your consent, your account statement and periodic account disclosures will be provided electronically.
Hardware and Software Requirements
To receive this information electronically, you will need:
- A computer or other device with Internet access
- Windows 95 or above with 64mb of RAM or more
- A personal e-mail address
- Adobe Acrobat Reader 5.0 or above - (download here)
Availability of paper records
If at a later date, you wish to obtain paper copies of your past statements or disclosures, you should:
- Call (616) 820-1444, or
- Write to us at Macatawa Bank, Statements, PO Box 3119, Holland, MI 49422, or
- Send us an e-mail to
- A $5 charge per duplicate paper statement may be imposed.
Withdrawing consent for electronic delivery
You have the right to withdraw consent for electronic delivery at any time. To withdraw consent and begin receiving a paper checking account or home equity line of credit account statements and disclosures, you should:

- Write to us at Macatawa Bank, Statements, PO Box 3119, Holland, MI 49422, or
- Send us an e-mail to
- A $2 charge per paper statement may be imposed.
